Assisted Living

Granite Gate Senior Living Community offers Assisted Living services tailored to meet the individual needs of our residents. Our philosophy of care is based upon resident choice, dignity, privacy, individuality and independence. We take a personalized approach, expanding our services when needed to meet the requirements of each resident right in their own apartment. A care plan is developed by our Health Services Director, the resident and their family, which coordinates a range of services to support the resident’s independence, self- direction and participation in decisions. Periodic reassessments are conducted by our Health Services Director.

Assisted Living Services May Include:

Personal Assistance

  • Clothing selection and dressing assistance both morning and evening
  • Escort assistance to meals and activities
  • Bath and shower assistance
  • Incontinence assistance
  • Personal hygiene assistance

Medication Management

  • Coordinating medications with physician and pharmacist
  • Medication administration

Food Service Assistance

  • Meal reminders
  • Assistance to and from meals
  • Tray service to apartment, upon request

Schedule/Transportation Coordination

  • Schedule medical appointments and local transportation
  • Planned outings and events
  • Grocery shopping and errands

Daily Housekeeping/Laundry

  • Bed making
  • Freshen up bathrooms
  • Change linens as needed
  • Empty trash
  • Launder personal clothing

Additional charges for Assisted Living services are based on a personal assessment with our Health Services Director.

I would like to thank you all for the care you are giving my mother. Before going on leave I did not know what to expect upon my arrival. In the last 10 days, I have seen how all of the residents have been cared for. As I return overseas to duty, this is one less concern I will think about.-David R.
